Organized healthcare is at your fingertips with this useful set of medical printables! The sheets can be used as is, but I highly recommend making a Medical Binder for you and/or your family. Also great if you are a caretaker for someone with a medical condition or if you’re caring for elderly parents. ::::::::::::::::: What You Get ::::::::::::::::: MEDICAL HISTORY (2 pages) – This complete form will allow you to note important information about your health including blood type, a record of immunizations, major illnesses, childbirth, etc. MEDICAL CONTACTS – Create a contact list for important medical professionals like your doctor, OB, dentist, emergency numbers, etc. DENTAL RECORDS – Keep track of all dental treatments on this sheet and you’ll never forget a tooth cleaning appointment!
